  • Certificate Copy (CA Special Driver Certificate-DL45)
  • DMV Printout (H6 dated with last 30 days )
  • Driver's License Copy (California Commercial Driver License : Class A or B with PS endorsements )
  • Other (Medical Examiner's certificate and First Aid card if required)
