*Ability to obtain and maintain a valid California driver's license Class A or B

• DMV“H6” Printout, Visit the local Lakeport DMV office, Request a ten year driver’s license printout called an “H6”, Provide your true full name or current Driver’s License Number, Pay the cost $ 5.00. (Please attach to your application.)
TO BE CONSIDERED FOR THIS POSITION, YOU ARE REQUIRED TO SUBMIT THE FOLLOWING ITEMS BEFORE YOU ARE OFFERED A POSTION WITH THE DISTRICT:
• Pass a Federal Motor Carrier Safety Administration (FMCSA) Title 49 Part 382 pre-employment drug test. Cost $69.50. Test location, ADTS Ukiah Office 419 Talmage Road, suite A Ukiah, CA 95482, Phone(707) 463-1230
• Pass a medical examination performed by a medical examiner approved by the FMCSA National Registry of Certified Medical Examiners. Cost $135.00 (Ask LUSD Transportation Office for locations (707 262-3022).
• You must also possess or be eligible to obtain a California Commercial Driver’s License, Class B with passenger and air brakes endorsements and a California Special Driver Certificate.
**Employee will be reimbursed after one year with the district.
