Position Summary:
Responsible for providing youth and families with Gang Reduction Youth Development (GRYD) prevention services with an emphasis on family support services and social learning for youth, ages 10-15 and their families.
Duties and Responsibilities:
- Provides outreach and prevention resources to local schools, parks, & community centers
- Conduct assessments to determine eligibly and needs
- Works closely with community stakeholders to promote community prevention efforts
- Creates and implements activities/workshops for youth and families.
- Provides a long term care plan for families which includes direct services and referral services, including, family-based case management, housing, educational support, mental health, substance abuse treatment, etc.
- Advocates for the client and family to address service gaps and barriers
- Works with parents/guardians/siblings to provide a positive support system for clients.
- Coordinates all activities for the client and families with various stakeholders in the person’s life including school officials, employers, probation officers, law enforcement officials, etc.
- Operates under the supervision of the Program Coordinator
- Maintains confidentiality and comprehensive case files.
- Monitors agencies to which clients are referred to assure quality of services.
- Works collaboratively with other case managers.
- Produces monthly program reports for the Program Coordinator
- Documents and inputs all services into mandated data tracking systems.
- Performs related duties as required.

Qualifications and Experience:
Any combination of experience and education likely to provide the required knowledge, skills and abilities, typically:
- Bachelor’s degree in a related field.
- 1 to 3 years of experience in related field.
