DEFINITION:
The Home Hospital Teacher will be assigned to a special education student and will possess the appropriate credentials as specified in the Individualized Education Plan. Mountain View Home/Hospital will assign Home/Hospital teachers in conjunction with SJUSD Personnel Services. During the period of Home/ Hospital instruction, the Home/Hospital teacher will be considered the teacher of record and will be responsible for the grading of the student and the submission of those grades to the school. The teacher is allowed one (1) hour of prep time per week of instruction. The minimum length of a home/hospital teaching session is one (1) hour and the maximum is 3 hours. Home/hospital teaching can be performed on any day except Sundays or holidays.
ESSENTIAL FUNCTIONS:
• Coordinate the instructional program with parents/guardians of the assigned student.
• Maintain an effective, ongoing working relationship with the student’s teacher(s) to obtain instructional materials, (i.e. textbooks, worksheets, tests, course objectives, pacing guides, and the appropriate report cards) and facilitate maintenance of an instructional program that is consistent with the program availability in the school setting.
• Meet with the principal and/or teacher(s) to provide appropriate information on progress made by the student upon completion of the home/hospital program.
• Coordinate an instruction program with the classroom teacher that assists the student in meeting the California content standards.
• Prepare lessons and materials.
• Administer district standards and benchmark tests to student.
• Complete IEP and any assessments due during the time student is on home/hospital
• Obtain student schedules and strategies from appropriate schools staff. The home/hospital teacher will coordinate with the appropriate personnel at the school site (identified by the principal, i.e. counselor, attendance clerk, etc.).
• Obtain parent/guardian signature after each home/hospital teaching session on appropriate attendance record.
• Submit required forms in a complete and timely manner to the principal

• Submit grades to the school for each student in each subject at the time of physician release from home/hospital teaching or the end of a grading period.
• Maintain all attendance records for each student and submit to Mountain View at the designated times.
• Maintain contact with parents/guardians.
• Access to reliable transportation.
WORKING CONDITIONS
Environment
• Office environment: constant interruptions.
• Outdoor environment: driving a personal vehicle to conduct work; visit school sites; travel to other organizations and companies; meetings and workshops.
Physical Abilities
• Hearing and speaking to conduct group training, exchange information in person electronically and on the telephone.
• Ability to read printed matter and computer screens.
• Ability to understand speech at normal levels.
• Dexterity of hands and fingers to operate a computer keyboard and other office equipment.
• Kneeling, bending at the waist, and reaching overhead above the shoulders and horizontally to retrieve and store files and supplies.
• Ability to lift and carry 30 pounds.
• Ability to climb stairs, walk, and sit for extended periods of time
• Ability to climb a ladder for purposes of inspection when necessary.
