Case Manager

Program: Gang Reduction and Youth Development

 

Position Summary:

 

Responsible for providing home-based services with emphasis on family support services and social learning through mentoring high risk youth, ages 14-18 and their families.

 

Duties and Responsibilities:

 

  • Maintains client files.
  • Creates and implements activities/workshops for youth and families.
  • Provides a long term plan of action for each clients which includes direct services and referral services, including, neighborhood-based case management, relocation and transition, educational support, referrals to housing, mental health, substance abuse treatment, and tattoo removal etc.
  • Advocates for the client when needed.
  • Works with parents/guardians/siblings and provides services to the family, if needed.
  • Coordinates all activities for the client and families with the various stakeholders in the person’s life including school officials, employers, probation officers, law enforcement officials, etc.
  • Coordinates with program coordinators as needed.
  • Maintains confidentiality and comprehensive confidential case files.
  • Monitors agencies to which clients are referred to assure quality service.
  • Works collaboratively with other case managers.
  • Coordinates and collates monthly program reports.
  • Performs related duties as required.*

Qualifications and Experience:

 

Any combination of experience and education likely to provide the required knowledge, skills and abilities, typically:

  • Bachelor’s degree in a related field.
  • Two (2) or more years of related experience.

  • Principles and practices of organization and administration
  • Principles of supervision, training and performance evaluation.
  • Personnel policies and procedures.
