JOB DESCRIPTION
JOB TITLE: Case Manager
DEPARTMENT: Gender Specific
STATUS: Non-Exempt
REPORTS TO: Program Coordinator
SUPERVISES: N/A
POSITION SUMMARY:
Responsible for providing wrap-around services with emphasis on providing job skills preparation to high risk youth, ages 14-25 and their families.
DUTIES AND RESPONSIBILITIES:
- Maintains client files.
- Creates and implements activities/workshops for youth and families.
- Provides a long term plan of action for each clients which includes direct services and referral services, including, neighborhood-based case management, relocation and transition, educational support, referrals to housing, mental health, substance abuse treatment, and tattoo removal etc.
- Advocates for the client when needed.
- Works with parents/guardians/siblings and provides services to the family, if needed.
- Coordinates all activities for the client and families with the various stakeholders in the person’s life including school officials, employers, probation officers, law enforcement officials, etc.
- Coordinates with program coordinators as needed.
- Maintains confidentiality and comprehensive confidential case files.
- Monitors agencies to which clients are referred to assure quality service.
- Works collaboratively with other case managers.
- Coordinates and collates monthly program reports.

QUALIFICATIONS AND EXPERIENCE:
Any combination of experience and education likely to provide the required knowledge, skills and abilities, typically:
- Bachelor’s degree in a related field(preferred)
- 1 to 3 years of experience in related field.
- 2 years’ experience working with at risk or probation youth.
