Job Summary

Job Summary

The Transition & Wellness Specialist provides comprehensive academic, career, transition, and behavioral health support services to students, particularly those identified as at-risk, underserved, justice-involved, or experiencing social-emotional and behavioral challenges. Working as part of a multidisciplinary care team under the supervision of a credentialed or licensed professional, the specialist promotes student success through prevention, early intervention, wellness education, transition planning, career readiness, and connection to community resources. The Transition & Wellness Specialist serves as a liaison among students, families, schools, community agencies, and behavioral health providers to ensure students successfully transition between educational settings, post-secondary opportunities, employment, and community based supports while enhancing overall well-being and resilience

Requirements / Qualifications

Education and Experience Associates degree in Social Work, Psychology, Human Services, Counseling, Public Administration, Education, Behavioral Health, or a closely related field. AND Two (2) years of experience providing academic support, case management, career counseling, wellness services, behavioral health support, youth development, or related services to children, adolescents, and young adults. OR Four (4) years experience providing academic support, case management, career counseling, wellness services, behavioral health support, youth development, or related services to children, adolescents, and young adults. Equivalent combinations of education and experience may be considered. Certified Wellness Coach I (CWC-I) or Registered-Certified Wellness Coach I certification, or ability to obtain certification within established timelines
