- CPR/First Aid Certification (If you do not have current certification, please include documentation to verify that you are willing to obtain this certification through training we offer.)
- Letter of Introduction
- Letter(s) of Recommendation (Please submit two (2) letter from current or prior supervisors/administrators.)
- Proof of HS Graduation (Copy of HS Diploma or HS Transcripts with graduation date)
