Applicants MUST hold a valid California Speech-Language Pathology Services credential OR Clinical Rehabilitative Services credential in Language, Speech and Hearing OR possession of a Master's degree in Communication Disorders AND a valid license from the Speech-Language Pathology and Audiology Board.

- Credential Copy (current copy of CA SLP credential with document number and expiration date)
