1. Letter of Introduction 2. Resume 3. Three recent letters of recommendation dated within the last 2 years – At least 1 from a current or former supervisor 4. Detailed valid CA Speech Language Pathologist Credential - - Attach a printout with the detailed Document Number from the CTC website (Do not attach the certificate) or status letter from credential analyst confirming credential recommendation 5. Transcripts (post-high school)
