**NOTE** Internal Applicants are required to submit a current Letter of Introduction, an updated Resume and any additional required documents (new or in need of an update) not on file. Substitutes must submit ALL required documents.

- Certificate Copy (Fundamentals of Coaching Education; Concussion; Sudden Cardiac Arrest and Heat Illness Prevention)
- CPR/First Aid Certification (Must have valid CPR & First Aid Certificate)
- Driver's License Copy (Must be valid)
- Letter of Introduction
- Letter(s) of Recommendation (Must Submit Two (2) Letters Dated within the last Twelve (12) months.)
- Proof of HS Graduation (or Equivalent)
- Resume
