Under the general supervision of the Program Coordinator II for the Homeless Education and Youth Program, this position supports the implementation and coordination of Enhanced Care Management (ECM) and Community Supports (CS) benefits. The role focuses on addressing the diverse needs of high-need youth and families by providing service coordination and comprehensive care management. The Family Case Specialist supports individuals and families with complex needs by assisting them in navigating healthcare systems and accessing housing resources. Responsibilities include addressing housing challenges, establishing connections with service providers, coordinating care, and tracking interventions and outcomes to ensure comprehensive support. This is a grant-funded position, subject to availability of funds.
Requirements / Qualifications
Required Qualifications: Education and Experience: Associate’s degree from an accredited college AND three (3) years of experience working with targeted population OR; Bachelor's degree from an accredited college or university AND demonstrated experience working with targeted population Licenses and Certifications: Possession of an appropriate, valid California Driver’s License and evidence of insurability Passing score on the MCOE Spanish language competency test
