Teacher Permit Copy Early Childhood Site Supervisor Letter of Introduction/Interest Resume Transcripts Two Letters of Recommendation signed and dated within the last 18 months TB Clearance TDaP - Tetanus, Diphtheria and Pertussis MMR - Measles, Mumps and Rubella Influenza vaccine (may choose to decline, but need a signed statement)
All required documents must be attached, if not your application will be screened out.
