- Fingerprint Clearance (Copy of Live Scan Fingerprint Clearance from Orange County Department of Education (A Certificate of Clearance is not valid for employment))
- Resume (Current Resume)
- TB Screening Result (Copy of TB Clearance Verification within the last 4 years)
