Get Safety Trained (Please see attachment on how to get safety trained certificates) California valid ID Social Security Card TB Clearance (valid within 4 years) High School Diploma/GED
- Driver's License Copy (or California valid ID )
- Other (Get Safety Trained Certificates )
- Proof of HS Graduation
- TB Screening Result
