Bus Driver Requirements, Please attach the following required documents: • CDL-Commercial License class A or B • DL-51 DMV medical form (Medical Examiners Certificate) • DL-45 California Special Driver Certificate • First Aid Card (if restriction on school bus certificate) • H-6 DMV Information Request (Driving History valid within 30 days) • T-01 (original copy) Training Certificate • T-02 (copy) Instructional Documentation
