Test Vaccination Record: MMR & TDAP Health Screening Report CPR/First Aide Must be eligible to obtain a valid CA Child Development Permit if you do not have one already. Please refer to Leaflet CL-797 on CTC.CA.GOV ( https://www.ctc.ca.gov/docs/default-source/leaflets/cl797.pdf ) Applicants must add any required or additional documents to their application as an attachment through Edjoin. Please do not mail, email, or fax documents to our office as they will NOT be included in your application.
