BASIC FUNCTION:
The job of the Community Schools Case Manager (CS Case Manager) is done for the purposes of coordinating and supporting programs that address the learning barriers for students at school sites. The CS Case Manager will develop a system of support for students, families, and community members by connecting families with community agencies and local government to improve student outcomes. The CS Case Manager is responsible for implementing systems and procedures, as well as monitoring and implementing projects. The CS Case Manager will ensure the California Community Partnership Program (CCSPP) grant is implemented at the school level.

REPRESENTATIVE DUTIES:
Essential Functions
Marginal Functions
- Train and educate all staff members on the California Community Partnership Program (CCSPP) grant objectives and the four implementation pillars.
- Conduct a needs assessment by working with school administrators, teachers, partner agencies, parents, and students to identify barriers to learning, available resources, and gaps in services.
- Identify, engage, and recruit partners to offer programs and services for students and families based on the community’s needs assessment.
- Develop the continuum of services for the students’ families and community members within the school neighborhood.
- Assist directly with information sharing and referral services to maintain an effective referral process to community resources.
- Develop, maintain, and publicize a schedule of programs and activities offered at the school.
- Create, strengthen, and maintain the bridge between the school and the community by creating opportunities for shared leadership and trust.
- Facilitate and provide leadership through collaboration in order to resolve related issues to service delivery, access, and coordination.
- Assess for improvement and shared accountability between partnerships.
- Advocate for students, families, and community members within the school neighborhood.
- Facilitate awareness of needs and trends within the community.
- Establish and coordinate service teams to ensure needs are being met and services are being maximized to the extent possible and to adjust as needed.
- Responsible for creating and implementing the CCSPP school’s plan.
- Coordinate all services that are part of the community school’s initiative that takes place during and beyond the school day. Services can include, but are not limited to, expanded learning opportunities, health services, mental health services, parent/family engagement, and direct material assistance.
- Assist, develop, coordinate, monitor, and implement the School Improvement Process via the development and implementation of the school-wide plan.
- Collaborate with school site Community Liaisons to align support services.
- Conduct required parent meetings related to the CCSPP school-wide plan.

INITIAL/MINIMUM QUALIFICATIONS:
Education:
- Bachelor’s degree in job-related field, preferably in social work, psychology, or sociology.
- Any other combination of education, training, and experience, which demonstrates that the applicant is likely to possess the required skills, knowledge, or abilities, may be considered.
- Three (3) years of increasing levels of responsibility, preferably in a K-12 public educational setting.
KNOWLEDGE, SKILLS, ABILITIES AND WORKING CONDITIONS:
SKILLS are required to perform multiple, technical tasks with a need to occasionally upgrade skills in order to meet changing job conditions. Specific skill-based competencies required to satisfactorily perform the functions of the job include: operating standard office equipment, using pertinent software applications, performing accounting procedures, planning and managing projects, and preparing and maintaining accurate records.
KNOWLEDGE is required to perform advanced math; read technical information, compose a variety of documents, and/or facilitate group discussions; and analyze situations to define issues and draw conclusions. Specific knowledge-based competencies required to satisfactorily perform the functions of the job include: budgeting practices regarding monitoring and control; principles of accounting and auditing; governmental accounting principles and practices; financial control and reporting; applicable sections of State Education Code; standard office practices and procedures; and pertinent policies, regulations, and/or laws.
ABILITY is required to schedule activities, meetings, and/or events; gather, collate, and/or classify data; and use basic, job-related equipment. Flexibility is required to work with others in a wide variety of circumstances; analyze data utilizing defined but different processes; and operate equipment using standardized methods. Ability is also required to work with a wide diversity of individuals; work with data of varied types and/or purposes; and utilize job related equipment. Problem-solving is required to analyze issues and create action plans. Problem-solving with data frequently requires independent interpretation of guidelines, and problem-solving with equipment is limited to moderate. Specific ability-based competencies required to satisfactorily perform the functions of the job include: perform and understand complicated math, statistics, and auditing techniques; analyze a variety of financial and statistical information; develop and maintain complex financial and statistical records; work under time constraints;
communicate with diverse groups; meet deadlines and schedules; set priorities; work with detailed information; interpret, apply, and explain laws, codes, rules, regulations, policies, and procedures; function in diverse, multi cultural, or difficult situations; and understand and work within scope of authority.
Responsibility
Responsibilities include: working under limited supervision using standardized practices and/or methods; directing other persons within a small work unit; directing the use of budgeted funds within a work unit. Utilization of resources from other work units may be required to perform the job's functions. There is a continual opportunity to impact the organization's services.

The usual and customary methods of performing the job's functions require the following physical demands: some lifting, carrying, pushing, and/or pulling, some stooping, kneeling, crouching, and/or crawling and significant fine finger dexterity. Generally, the job requires 90% sitting, 5% walking, and 5% standing. This job is performed in a generally clean and healthy environment.
