Job Summary

Position Overview  Under the supervision of the Family Preservation Clinical Coordinator, the social worker is responsible for carrying a case workload of 10-12 cases at a time in a manner consistent with best practices. Duties Conducts intakes and evaluations of clients. Coordinates, disseminates and collects the outcome evaluation instruments as required by Contract. Works with clients following their initiatives in the development and achievement of their case plan goals. Conducts visits to homes, schools or community facilities in order to help client achieve their treatment goals.  Participates in the ongoing quality assurance efforts for the provision of Family Preservation Services to clients. Participates in staff weekly meetings, case conferences, training and professional consultation as necessary. Participates in weekly individual and group supervision Follows states laws regarding confidential information in verbal and written communication. Works collaboratively with members of the client's treatment team such as clinician, psychiatrist and psychologist. Participates in multi-disciplinary case conferences to develop initial treatment plans as well as to review the clients progress and establishing the most appropriate case plan goals. Maintains and seeks communication with community groups and resources which support the clients psychological functioning. Prepares reports and recording in a timely manner - (such as FAF assessments, weekly progress notes, monthly and termination reports) Performs other job-related duties as directed by supervisor. Additional duties Carries out mission, vision, and values established by the President/C.E.O. and Board of Directors Participates in the overall agency CQI process Must have the ability to nurture respectful relationships with clients, children and must be sensitive to the clients cultural and socioeconomic characteristics.

Requirements / Qualifications

Qualifications and Experience Requirements  MSW or MA in related field. A Bachelor’s degree in Social Work or closely related field in combination with relevant years of experience will be considered in lieu of advance degree. At least three (3) years in case management experience working with low-income children and families and staff supervision. Special Skills/Knowledge/Abilities Strong leadership skills desired with a solid foundation on human development and family dynamics A clinical Professional background is also highly desirable A keen understanding of the African American and Latino communities Must have the ability to nurture respectful relationships with clients, children and must be sensitive to the clients cultural and socioeconomic characteristics Experience with developing and implementing/coordinating children and family programs Organized, multi-task, detail oriented Able to prioritize, to work independently and as team member Experience in providing high quality training in the areas of family dynamics, and strength based models. Bilingual/bicultural required Special Requirements Must have a reliable automobile for use on the job (mileage to be reimbursed).  Valid California Driver License.  State required automobile insurance coverage. TB clearance, to be renewed every two years.
