1. Do you have a fever (or a temperature over 100 degrees Fahrenheit)?
2. Do you have a new cough that cannot be attributed to another health condition?
3. Do you have shortness of breath that you cannot attribute to another health condition?
4. Do you have a sore throat that you cannot attribute to another health condition?
5. Do you have muscle aches that you cannot attribute to another health condition or that may have been caused by a specific activity (such as physical exercise)?
6. Have you been in close contact with another person with a lab-confirmed case of COVID-19 in the past 14 days? (Close contact is currently defined by the CDC as having been within 6 feet or fewer for 15 minutes or more within a 24-hour period.)
7. Have you been instructed to self-quarantine by a doctor or another health provider or authority in the past 14 days?
If any of the answers to the self-assessment are “yes”, do not report to the testing location and contact Human Resources for an alternative testing date.
