2. Do you have a new cough that cannot be attributed to another health condition?
3. Do you have shortness of breath that you cannot attribute to another health condition?
4. Do you have a sore throat that you cannot attribute to another health condition?
5. Do you have muscle aches that you cannot attribute to another health condition or that may have been caused by a specific activity (such as physical exercise)?
6. Have you been in close contact with another person with a lab-confirmed case of COVID-19 in the past 14 days? (Close contact is currently defined by the CDC as having been within 6 feet or fewer for 15 minutes or more within a 24-hour period.)
7. Have you been instructed to self-quarantine by a doctor or another health provider or authority in the past 14 days?
If any of the answers to the self-assessment are “yes”, do not report to the testing location and contact Human Resources for an alternative testing date.

Tuesday, August 10, 2021, at 1:00 p.m. SHARP. Education Center, Board Room, 4850 Pedley Road, Jurupa Valley, California 92509. THE TESTING WILL TAKE APPROXIMATELY TWO (2) HOURS TO COMPLETE. PLEASE BE PREPARED TO REMAIN IN THE TESTING ROOM FOR THE ENTIRE TEST PERIOD. THIS IS THE ONLY NOTICE OF THE TESTING.
IMPORTANT INFORMATION
YOU WILL NEED TO BRING A VALID STATE OR GOVERNMENT ISSUED ID IN ORDER TO TEST. PLEASE ALLOW SUFFICIENT TIME TO PARK AND CHECK IN WITH THE FRONT DESK PRIOR TO ENTERING THE TESTING ROOM.
