- Certificate Copy (Copy of Certificate of Clearance printed from www.ctc.ca.gov)
- Copy of Transcript (BA/BS degree must be conferred)
- Driver's License Copy (Clear copy of valid (non expired) California Driver's license)
- Letter(s) of Recommendation (Two (2) current, dated and signed within the last 2 years )
- TB Screening Result (Copy of TB test results dated within the last 4 years)
- Test Results/Materials (CBEST Results.)
