- TB Screening Result (TB clearance results must be dated within the last 60 days of submitting application.)
- 30-Day Substitute Teaching Permit OR Child Development Permit - Teacher (Child Development Teacher Permit (or Higher Permit))
OR Multiple Subject Teaching Credential - General Subjects
OR Single Subject Teaching Credential - Other
