*Any combination equivalent: Bachelor's Degree in Occupational Therapy or related field. *Valid Occupational Therapist Certification issued by the American Occupational Therapy Certification Board. *Valid State Occupational Therapist License required. Only completely filled application will be considered. All fields on the work experience section must be completed. A resume will not be accepted in lieu and your application will be disqualified.
