POSITION SUMMARY The YouthBuild Case Manager will be part of an innovative team of service providers and educational experts that will work collectively to operate an innovative Program for young people in the San Bernardino region that faces the barriers to high school completion and finding living wage employment. The YouthBuild Case Manager will provide one-on-one and small group supportive services to at-risk and transition-age youth/young adults who are preparing to join the workforce. The YouthBuild Case Manager will assess basic needs, identify barriers to education and employment, develop an Individual Service Plan to address those barriers, and coordinate services to help youth/young adults prepare for and secure living wage jobs with a career path upon graduation of the YouthBuild Program.

PRIMARY FUNCTIONS
Responsible for providing case management in-take, assessment, planning, monitoring, and follow-up of the YouthBuild Program. Services are provided for youth and young adults. Services are provided in an integrated educational setting, of multi-disciplinary para-professionals and professionals with a person-centered, family-centered, client-oriented approach to services.
MINIMUM QUALIFICATIONS
Education/Experience:
Bachelor’s degree required from an accredited institution of higher education in fields of study such as social work, sociology, psychology, counseling or related field, or meets equivalent qualifications as established by the personnel procedures of the PESI using a combination of education and additional years of related work experience as established by the agency’s personnel office.
Four years of full-time work experience in an educational/employment development setting implementing services and activities to assist low-income young people, out of school youth, youth in the foster care system, homelessness youth and young adults, youth who have had difficulties with the justice system and are in need of life and career assistance. Applicant has demonstrated ability to successfully work with potential clients.
Experience working with diverse ethnic and economic communities; knowledge of local community resources and social service providers. Demonstrate respect and non-judgmental attitudes toward clients in all contracts with community professionals and agencies.
• Excellent oral and written communication skills including public speaking. Spanish-English Bilingual/Biliterate is desirable. Ability to write clear, accurate log notes, and complete other paperwork thoroughly and accurately. Ability to gather information and assess client situation rapidly and accurately. Knowledge and experience with software programs including but not limited to MS Word, Adobe PDF, PowerPoint, database management, desktop publishing, and use of electronic media, i.e., Facebook, Twitter, Linked-In, group texting and website management.
JOB RESPONSIBILITIES
1. Contact, receive and review referrals to the YouthBuild Program from local schools, community-based organizations, and government agencies, including walk-ins. Participate in and coordinate outreach activities that include making presentations to non-profit organizations, government agencies, probation/parole staff and agencies, and school outreach in the targeted neighborhood.
2. Assist in the planning and implementation of the YouthBuild Program orientation (Mental Toughness Program) and Intake documentation.
3. Conduct individual needs assessments with regard to access to basic needs such as housing and food; education level and school enrollment; current income, employment, and past work experience; gang affiliation; and involvement with the justice system leading to eligibility for YouthBuild Program services.
4. Assist youth to enroll in PAL Charter Academy to complete a high school diploma, general equivalency degree, and/or postsecondary enrollment as needed.
5. Provide project workshops to facilitate employment readiness, leadership, and life skills.
6. Continuously assess and evaluate referral resources to determine their appropriateness for each client.
7. Track participation in each Project component (educational, construction, counseling, leadership development and employment readiness, and life skills development).
8. Meet with clients on an individual basis at least bi-weekly to monitor progress in each Project component; identify resources to meet supportive service needs; problem-solve; review the Individual Service Plan (average case load is 45 clients) and modify as needed.
9. Assist with placement in construction apprenticeships, construction sites, construction plus sites, and other employment opportunities.
10. Build relationships with community-based organizations and government agencies to promote collaboration, solve problems, and to identify resources for clients.
11. Provide information & referrals for youth and young adults who cannot participate in the Project due to eligibility criteria or limited Project capacity.
12. Follow PESI and YouthBuild procedures with regard to recordkeeping, documentation, confidentiality, and your ability to use and maintain paper and electronic case files.
13. Meet weekly with YouthBuild staff to plan activities, to strategize on service delivery for clients, and to support other activities identified by PESI.
14. Assist with fulfilling the DOL YouthBuild SOW requirements and YouthBuild Program Manual.
15. Report to YouthBuild Project Director.
Other Requirements:
Travel is a component of the position with local and regional travel using personal transportation, therefore a reliable vehicle is required. Must have a California driver’s license and good driving record with no major violations, and proof of automobile liability insurance meeting PESI policy standards. Driving record will be requested to be provided from DMV.
Employment subject to clearances, including background check, finger printing check, tuberculosis testing, and drug testing.
SALARY: Based on qualifications and salary range of position type.
