*Any combination equivalent: Bachelor's Degree in Occupational Therapy or related field. *Valid Occupational Therapist Certification issued by the American Occupational Therapy Certification Board. *Valid State Occupational Therapist License required. Mark field that are “not applicable” as “N/A”.
