*California Teaching Credential or 30 Day Substitute Permit *Sign up as a Substitute with SCOE (Sonoma County Office of Education) 707-524-2794 *After you've completed the process at SCOE complete this application. *After you have submitted your application, bring copy of your I-9, DE-4 & W-4 to SRCS District Office-No appointment needed. Bring your original driver's license & social security card.

      >SIGN UP AT SCOE PRIOR TO COMPLETING THIS APPLICATION. > After you've submitted the application, bring your driver's license, social security card, W-4 Form, DE-4 Form, I-9, Emergency Contact Form, Calstrs Form (if applicable) to the District office to sign up on our Substitute List. If you want direct deposit, bring a Direct Deposit form and a voided blank check. No appointment is necessary. You may come in to sign up Monday-Friday (9:00am - 11:30 & 12:30 - 4:00pm).
