- DMV Printout (H6 report issued less than 30 days prior to application)
- Driver's License Copy (Valid Class B License)
- Health (Medical Certificate)
- Other (California Special Driver's Certificated - issued by the California Highway Patrol)

• A valid First Aid and CPR Certificate (may be required)
Successful applicants must obtain and/or provide the following before beginning employment:
Fingerprint clearance from the Department of Justice and/or FBI
Proof of Tuberculosis clearance
Social Security Card for payroll purposes
Proof of eligibility to work in the United States
Proof of education
Pre-Employment Physical (Scheduled by HR)
